Looking at the photo, the long bracket is not included. Nor are the 4 short bolts and nuts. What you get is the 'U' shaped fork, two long bolts and two nuts. It is designed to fit on their stableloader, which it does. BUT, the nuts will fall through the holes of said bar, so you will need to supply your own 1/4" washers, I used 4. Personally, for what you get, it is over priced; a formed sheet metal strip and 2 bolts. Really, 20 bucks? But if you want it to carry that wheel rather than put it inside your truck, it works great, just make sure you 1) have it far enough away from where the bike forks fit to the bike hitch or your wheel will hit the handle bars of your bike, and 2) is at least 14" away from the rear of your truck for the wheel to clear, depending on your wheel size, as mine are 700's.

Another point to note, my truck bed (Dakota) just barely allows my road bike to fit with the rack positioned as it is, just far enough back from the rear of the truck for the wheel to fit in the wheel hitch without hitting the truck, and with the bike attached to the stableloader, with an inch to spare at the tail gate.

Overall, it does the job, but you will need extra hardware and not get everything as shown. The bolts also are not stainless, just coated, so be aware that you MIGHT want to replace those too. They will rust after a while.
